How to mail-in your glasses
- Download, print, and fill out a repair request form by clicking on the link below
- Please include a street address for return UPS shipping and a contact phone number.
- Safely package your broken glasses in a box and include the completed Repair Request Form.
- Use an insured method to ship your glasses to PRO-OP. We suggest UPS, FedEx, or USPS.
- Once we receive your eyeglasses, our office staff will contact you by phone with a repair quote.
- Payment can be made by Check or VISA / MasterCard and must be tendered before repairs begin.

If you would like to use the drop off option for your glasses:
•Go to The UPS store at 1520 West El Camino Ave, Sac, CA 95833.
•Tell the UPS staff you have glasses for Pro-Op.
•They will hand you a box for your glasses.
•Fill out the form inside and hand back to them.
•UPS charges a handling fee of $6 each way for drop off & pickup service.
•We add the fees to your bill later, as UPS does not accept payment for PRO-OP services.
